RESUMO

Neste vídeo, mostramos a técnica cirúrgica do buraco macular combinada com a extração da catarata de paciente do sexo feminino com buraco macular grau IV visto à tomografia de coerência óptica. Realizamos a facoemulsificação com implante de uma lente intraocular de acrílico dobrável no interior do saco capsular. Utilizamos trocáteres 23 gauge para acesso ao interior do bulbo ocular com os instrumentos cirúrgicos. A vitrectomia é feita desprendendo todo o gel vítreo dos seus pontos de maior adesão e um corante vital - “azul brilhante” - é utilizado para melhor identificação da membrana limitante interna e sua remoção. A seguir, fazemos a troca fluido/ar e ar/SF6 20% para promover o fechamento do buraco macular. Por se tratar de incisões auto-selantes, ao remover os trocáteres, realizamos uma leve compressão no local das incisões facilitando, assim, o seu fechamento.

ABSTRACT

In this video, we demonstrate the surgical technique for macular hole combined with cataract extraction performed in a female patient with a stage IV macular hole revealed by an optical coherence tomography. We performed phacoemulsification with an intraocular lens implant made of foldable acrylic within the capsular bag. We used 23-gauge trocars to access the inside of the vitreous cavity. Vitrectomy was performed by detaching the vitreous gel from its most adhesive points. Brilliant Blue, which is a vital stain, was used to better identify the internal limiting membrane and to perform the peel. Next, we performed the air-fluid and air-sulfur hexafluoride (sulfur hexafluoride 20%) exchange to aid the closure of the macular hole. As this was a case of sutureless incisions, after removing the trocars, we lightly compressed the area of the incisions to ensure their closure.
